The Joint Commission recently published the 2018 National Patient Safety Goals. They have a 9 page document and a single page summary.

In short:

Identify patients correctly- at least 2 ways to identify patients; make sure patients receive correct blood during transfusions.

Use medicines safely- label all medications before procedures; record and sign out the correct information about patient’s medications, including home and currently receiving medications.

Prevent infection- wash hands according to CDC or WHO guidelines; use proven guidelines to prevent infection after surgery.

Prevent mistakes in surgery- correct surgery on correct patient on the correct place on the patient’s body; mark the correct place before surgery; time out before surgery.
